چکیده

Rhythmic palatal myoclonus is a neurologic sign with one of the most precise pathologic correlations. It almost always is associated with hypertrophic degeneration of the inferior olivary nucleus [1] ; however, such hypertrophic degeneration is not always accompanied by palatal myoclonus. The abnormality of the olivary nucleus is thought to be the result of a transsynaptic degeneration due to a lesion either in the ipsilateral central tegmental tract or in the contralateral dentate nucleus. We present a case that we think illustrates the MR detection of hypertrophic degeneration of the inferior olivary nucleus f2].

Hypertrophic olivary degeneration following pontine haemorrhage: hypertensive crisis or cavernous haemangioma bleeding?

The clinical and magnetic resonance (MR) features of hypertrophic olivary degeneration are described, along with a rare but treatable cause of this entity-pontine cavernous haemangioma. Hypertrophic olivary degeneration occurs after focal lesions to the dentato-rubro-olivary pathway, typically following a pontine haemorrhage involving the ipsilateral central tegmental tract, the contralateral s...

Palatal myoclonus--a case report.

Palatal myoclonus is usually due to a brainstem or cerebellar lesion disrupting the dentato-rubro-olivary pathway. Rarely it may be caused by a cortical lesion. The precipitating factor in 70% of all cases is an infarct. We describe an unusual case of a patient with palatal myoclonus who had an old ipsilateral cerebellar infarct and a new contralateral subcortical (corona radiata) infarct. We p...

Cerebellar MR changes in patients with olivary hypertrophic degeneration.

PURPOSE To describe MR changes in the cerebellar cortex and the dentate nucleus in patients with inferior olivary nucleus hypertrophy. METHODS MR scans of 11 patients with palatal myoclonus were reviewed. Among them, we selected 5 cases that showed lesions in the central tegmental tract and the ipsilateral inferior olivary nucleus. We evaluated MR changes of the cerebellar cortices and dentat...

Clinical and magnetic resonance imaging manifestations of Holmes tremor.

Holmes tremor is a rare symptomatic slow tremor in the proximal parts of the limbs. It may be present at rest or maintenance of a posture, or during the movement of the affected limb. We describe herein three patients of Holmes tremor with possible etiologies of brainstem infarction and head injury. The intervals between the causal events and the appearance of tremor range from 1 month to 12 mo...

Teaching NeuroImages: hemorrhagic cavernoma with secondary development of hypertrophic olivary degeneration.

Hypertrophic olivary degeneration (HOD) is secondary degeneration of the inferior olivary nucleus (ION) due to a primary lesion in the dento-rubro-olivary pathway. This pathway is known as the Guillain and Mollert triangle, containing the dentate nucleus and the contralateral red and inferior olivary nuclei (figure e-1 on the Neurology® Web site at www.neurology.org). The commonest presenting s...
